I do hereby grant to Mike Rowe and mrWF Holdings and their assignees and licensees (including without limitation CNN and its affiliates) and their respective officers, employees, agents, affiliates, affiliated websites and contractors (together referred to hereinafter individually and collectively as “mrW”) the right to use submitted photographs, videotape or digital recordings, and to use my, and any other person appearing in the materials, identity, name, likeness, image, sound of my voice and title in the video (“Video”) submitted by __________________________________, (together collectively referred to as “Submitted Material”) for any purposes, including, without limitation, on and/or in connection with the mikerowe.com website and other websites affiliated with mrW, as well as in connection with the promotion of mrW and any mrW entertainment properties (including Somebody’s Gotta Do It) and/or the furtherance of mrW’s business generally, in any manner, in any and all media or medium whether now known or to be developed in the future.  I hereby agree to all the Terms and Conditions found at www.mikerowe.com/home/terms-and-conditions

I understand that the Submitted Material may be edited, copied, exhibited, published or distributed and waive the right to inspect or approve the finished product in which the Submitted Material appears. I waive any rights, claims, or interest I may have to control the use of my identity or likeness in whatever media used. I understand that there will be no credit given to me or compensation paid to me and no financial or other remuneration will be provided relating to this release, either for initial or subsequent use.

I also understand that mrW is not responsible for any expense or liability incurred as a result of my participation, including medical expenses due to any sickness or injury incurred as a result. I hereby release any and all claims against mrW and any person or organization utilizing, for any purpose, the material described in this release.

I warrant that I am at least eighteen (18) years of age (or, if not, that my parent or legal guardian has agreed to the foregoing and signed below) and that I have the full, complete and unrestricted right and authority to enter into this Release. I acknowledge that the Assignees will rely on this permission potentially at substantial cost to them and hereby agree not to assert any claim of any nature whatsoever against anyone relating to the exercise of the permissions granted hereunder.

CONSENT OF PARENT OR GUARDIAN (IF APPLICABLE)

As the parent or guardian of the minor named above, I hereby expressly approve and consent to this Name and Likeness Release and waive all obligations and rights which I may have in connection therewith. For good and valuable consideration, I hereby guarantee that the minor shall not at any time disaffirm the Name and Likeness Release by reason of the minor’s minority or otherwise and hereby agree to indemnify the Assignees and each of them from any and all loss arising from or relating to any claims made by or on behalf of the minor relating to the subject matter of this Name and Likeness Release or from any and all loss arising from or relating to the breach of any guarantee(s) made by the undersigned in the parental consent. The consideration referenced in this Name and Likeness Release shall be deemed to be the consideration in full for all rights granted to the Assignees and for all obligations undertaken pursuant to this Name and Likeness Release and pursuant to this parental consent, not only by the minor but also by the undersigned.
